Below is a list of Downloadable Registration forms.  You can download the form you need to your computer or the file has opened you can use the print command to print a hard copy of the form you desire.

Each Program requires both a registration form and a medical release form.

The forms require Adobe Acrobat Reader installed on your computer.  This is available free of charge at the Adobe Web Site.

If you are having problems opening the document directly in your browser through Acrobat Reader you might try downloading the document to your hard drive and then opening the file in Acrobat.  

To do this on your PC, simply right click on the document you would like to download.  A menu box should open.  Select from the menu "Save Target As" and another window should open offering you a chance to choose the location where you want to save the .pdf file.  Choose the file location in which you will save the document.  Once downloaded you can start Acrobat Reader and simply open the document now saved to your hard drive.

Mac Users:  There may be some difficulty opening this link if you have an older version of Acrobat Reader.  In the event you click on the link and get only a blank page, hit your browser's "back" button; click once on the link, hold down your mouse button, and drag the link to your desktop; open your Acrobat Reader version; in the pull-down menu under "file" choose "open"; open the document from your desktop.
